Phyllis J. (Forney) Carr, 79, of Chestnut Towers Apartments, Sunbury, passed away at 10:50 p.m. Wednesday, Jan. 29, 2020, at the Williamsport Hospital.

She was born April 17, 1940, in Mahoning Township, Montour County, a daughter of the late Charles H. and Blanche (Jackson) Forney.

Phyllis resided in Sunbury since 1983 and previously lived in Riverside most of her life. She was a 1958 graduate of Danville High School and a 1968 graduate of Danville Area School District School of Practical Nursing.

Phyllis was employed by the Sunbury Community Hospital as a licensed practical nurse for 27 years, working mainly in the emergency room.

She was a faithful member of the Amazing Grace Community Church, Northumberland, where she assisted in many activities and helped in cleaning the church.

Phyllis enjoyed attending craft shows with her mother for more than 40 years.

She is survived by two daughters, Tammi J. Jimenez and husband Ramon of Sunbury, and Yvonne Miles of Hazleton; three stepchildren, Ronald Mull and wife Sharon of Sunbury, Doris Heemer and husband David of Trevorton, and Floyd Carr Jr and wife Jean of Middleburg; four grandchildren; 13 stepgrand-children; five great-grandchildren; 10 stepgreat-grandchildren; and one sister, Roberta Enterline of Florida.

She was preceded in death by one stepdaughter, Lynn Reed.

A memorial service will be held from 10 to 11 a.m. Saturday, Feb. 8, at Amazing Grace Community Church, 138 Leed St., Northumberland, with Pastor Darren Hunt officiating.

Arrangements have been entrusted to the care of the Roat-Kriner Funeral Home, 1133 Bloom St., Danville.
